Laser coagulation refers to a type of laser surgery that uses light in the green spectrum to force tissues to clot, or coagulate.

Once the tissue loses its blood supply, it shrinks and will eventually be reabsorbed by the body. The major advantage of this type of surgery is that it can usually be completed in an outpatient setting with minimal discomfort for the patient. In addition, the risks of blood loss and infection are heavily diminished because the laser simultaneously closes off the blood vessels and sterilizes the area during the procedure.
A subtype of this surgery, photocoagulation, is the type of laser surgery employed in most eye surgeries. This type of eye surgery is considered to be far superior to previous methods. In the case of a torn or detached retina, for example, the physician often uses an argon laser through a special contact lens in order to "weld" the retina to the underlying eye tissue.
Photocoagulation is considered the best treatment approach for diabetic retinopathy as well and can reduce the chances of severe vision loss by 50 to 75% if performed in the early stages of the disease. Other eye conditions that can be successfully treated with photocoagulation include macular degeneration, retinal ischemia, retinal detachment, and ruptured blood vessels in the eye.
Prior to laser coagulation, the only surgical treatment for severe varicose veins was vein stripping. This procedure required the surgeon to make numerous closely-spaced incisions along the legs and to strip the veins in each section. Despite a painful recovery period lasting several weeks, vein stripping was rarely successful and often required additional procedures. Laser coagulation, on the other hand, requires one or two small incisions that are usually made at the knee or groin. It can also be performed on an outpatient basis, has a much shorter recovery period, and rarely requires additional surgery.
Laser coagulation is not limited to use in optical and venous surgeries. Some types of cancer have also been successfully treated by focusing the laser on the blood vessels that supply the tumor with oxygen and nutrients. The tumor will then begin to shrink and will eventually be reabsorbed by the body. Although the use of lasers in the treatment of cancer is promising, many factors are considered before attempting the use of lasers to treat some types of tumors. The use of laser coagulation procedures is increasing in many others branches of medicine as well.

Laser for busting blood clots

Indian researchers have developed a novel therapy to bust blood clots using laser, which they say can be combined with low concentrations of traditional drugs to reduce life-threatening complications and effectively treat cardiovascular diseases and blood clot-related disorders.
Based on the principle of photothermal therapy, the technique harnessed heat generated by exposing gold nanorods (GNR) to near-infrared (NIR) laser. It specifically targeted blood clots and produced heat locally to chip away the obstruction.
"Our method uses targeted-GNR which specifically binds to the clot (thrombi) and generates local photothermal heat upon NIR-laser exposure.
Arterial or venous clots lead to devastating consequences like heart attack, stroke or pulmonary thromboembolism. This has been the first attempt to apply the principle of photothermal therapy to ablation of clots," Debabrata Dash, Professor, Department of Biochemistry, Institute of Medical Sciences, Banaras Hindu University, said.

"Thrombus is a jelly-like mass occluding blood vessels, which is composed of polymerised fibrin strands (fibrin is a protein) held together mostly by non-covalent forces. We reasoned that if we apply local heat to thrombus the component fibrin strands will be separated and will be washed away from the lesion site by the flow of blood," Dash, the lead researcher of the study, said.
The researchers deployed mice as a test bed for their reasoning.
The currently available therapy for busting blood clots is medication with tissue plasminogen activator (tPA), streptokinase or urokinase. It is however, fraught with bleeding complications, pointed out Dash.
"Administration of these drugs is associated with serious life-threatening complications, including severe haemorrhage and thus requires consistent physician supervision and monitoring. Secondly, these medications are not directed at a localised clot and therefore, lead to a generalised effect. This represents another major limitation of traditional thrombolytic therapy," Dash said.
But the new method circumvents the limitations. It is specific and enables usage of lower doses of the traditional drug.
"Our method uses targeted-GNR which specifically binds to thrombi and generates local photothermal heat (that breaks down the non-covalent interactions) upon NIR- laser exposure. In combination with photothermal heat, we administered streptokinase at a dose which is significantly lower than the therapeutic dose. Combining it with streptokinase therapy abolishes the life-threatening side effects associated with streptokinase," Dash said.
What intrigued the scientists more was the synergism between the two techniques.
"In mice, the light therapy in combination with a low-dose streptokinase cleared blood clots more efficiently than laser therapy alone. NIR laser has a penetrating power up to 10 cm. Hence, it can effectively access and ablate any intravascular thrombus within this range," Dash said, adding the blood flow in the vessel was slowly restored following the application of the radiation.
They claimed they came across "no major side effect of this procedure”.
"The heat generated by laser exposure did not harm the blood vessel in mice. Gold nanorods are relatively safe for human use. They have been widely suggested for intravenous administration in order to target cancer cells. Photothermal therapy is a non-invasive mode of treatment," Dash said.
To take it forward, the team intends to target clots in non-human primate model.

The effect of red blood cell aggregation and how it can be reversed

The blood profile reveals a person’s state of health. If we look at a blood sample of an unhealthy subject, we will see that the red blood cells (RBCs) are sticking together (aggregating) and behaving like glue – with high resistance to flow (“high viscosity”).
Under a microscope, the blood sample of an unhealthy subject would look something like in figure 1:

With the same person in a healthy condition, his blood sample would look entirely different, as in figure 2 below: 2
Figure 2: Blood sample of a relatively healthy person Visually, it is clear that the main difference between the two samples is how the red blood cells (RBCs) or erythrocytes are seen to be aggregated in figure 1 and disaggregated in figure 2. It is not possible to demonstrate from a static picture like this but when the blood samples were taken, the blood in figure 2 was also observed to be less viscous.
“Blood aggregation” was coined as early as 1786 by health knowledge leaders. Occasionally it is called “blood sludging”. Whatever the phenomenon is called, it has been associated with many diseases.iThis may surprise many: the two samples above are actually “before” and “after” pictures of blood samples taken only 30 minutes apart from the same subject after using a device like the Qi-Light by RadiantLife as shown in Figure 3 below. Effect of RBC aggregation on blood circulation and tissue perfusion The following is extracted from a review of evidence-based work on RBC aggregation by Baskurt and Meiselman.ii
Effect of RBC aggregation on microcirculatory blood flow
In studies employing intravital microscopy it has been observed that intensified RBC aggregation increases microvascular flow resistance.iii
RBC aggregation in arterial and/or capillary microcirculation would affect the bulk viscosity of blood in larger blood vessels and the increased energy cost of disaggregation at the entrance of microcirculation.
RBC aggregation has also been found to decrease the density of functional capillaries, resulting in decreased overall microcirculatory blood flow. This is found to be especially significant under reduced arterial pressure.v Enhanced RBC aggregation also fills capillaries with non-flowing RBC making these capillaries dysfunctional.vi
Effect of RBC aggregation on venous flow resistance
The venous side of the circulatory system returns deoxygenated blood back to the heart and lungs to discharge the waste and carbon dioxide. Blood flow in the venous circulation is characterized by lower shear rates (change in velocity of blood flow parallel to vascular walls) compared to the arterial side. For this reason occurrences of RBC aggregation are more significant in the venous circulation areas.vii
Experiments show that normal RBC aggregation significantly contributes to venous vascular resistance in resting muscles and plays a significant role in vascular system homeostasis (stable bodily state).
viii It has also been demonstrated that RBC aggregation affects velocity profiles in venous blood flow, especially under reduced flow rates.ix The energy loss caused by RBC aggregation contributes to flow resistance. Effect of RBC aggregation on whole organ perfusion
A study isolated a rat’s heart and then perfused it with a different concentration of dextran (a compound often used by researchers to simulate RBC aggregation). The results suggest that relatively low levels of RBC aggregation may reduce flow resistance in the organ, whereas greatly enhanced aggregation greatly increases resistance to blood perfusion to the organs.xEffect of RBC aggregation on tissue hematocrit At the tissue level, tissues are also adversely affected when they are not properly provided with nourishing blood. They are served by the network of capillaries; and as discussed, if the flow is impeded by RBC aggregation, their condition will be adversely impacted.
There are however, specific studies that look into tissues: more specifically on hematocrit (the ratio of blood volume 4occupied by RBC). Tests demonstrate that alterations in RBC aggregation induced by fibrinogen infusions affect tissue hematocritic values in rat myocardium (muscular middle layer of the wall of the heart).xi These experiments were also repeated using more recently developed poloxmer coating method to alter RBC aggregation. These confirm that enhanced RBC aggregation alters myocardial tissue hematocrit values.xii
In summary there is substantial evidence to show that RBC aggregation impedes blood flow. The presence of RBC aggregation affects microcirculatory blood flow, venous flow, whole organ perfusion, and tissue hematocrit. As mentioned earlier, the presence of RBC aggregation is commonly attributed to inflammation which then stimulate the release of fibrinogen into the blood circulation system. When inflammation is reduced through LLLT, the level of fibrinogen in the blood will also be reduced. As the result RBCs will be visibly disaggregated. The photodissociation theory
Tests on rabbits established that hemoglobin is a primary photoacceptor absorbing low-intensity laser radiation of light of red and infra-red (“IR”) wavelengths. The exposure of blood to this radiation causes clearly defined changes in the IR and visible absorption spectra of the blood and erythrocytes (red blood cells). These spectral changes arise as a result of partial photodissociation (breaking down of chemical compounds with light) of hemoglobin-ligand (substance to bind biomolecules) complexes in the process of absorption of laser radiation. It is suggested that photodissociation is a primary reaction that arises in blood exposed to a low-intensity laser radiation.xix This result is the disaggregation of aggregated RBC. The transient local heating hypothesis
Upon contact with blood, a substantial amount of absorbed red light energy is converted to heat, which causes a local transient increase in the temperature of absorbing chromophores (molecules 5responsible for their colours). This local transient heating of absorbing molecules is different from the general heating of the whole cell, tissue and organism as commonly imagined. The local transient increase in temperature causes structural changes, and trigger biochemical activity that results in the disaggregation of RBC.xx
The negative surface charge theory
Some researchers suggest that LLLT raises and restore the negative surface charge of the RBCs, rebalancing the play with electrolytes, and restoring their natural state of the RBCs repelling each other.xxi
In summary, there are several theories as to how LLLT would neutralise RBC aggregation. What matters is that LLLT is proven to succeed in disaggregating RBC. From stasis to red blood cell aggregation Recent research also shows that stasis in the vein is associated with RBC aggregation.xxii
The aggregation is found to be due to the presence of macromolecular proteins such as fibrinogen and globulin.xxiii Fibrinogen is the protein which is sensitive to inflammation and helps with the blood clotting function. It is a dominant factor in RBC aggregation.xxiv Since inflammation is caused by an illness or disorder, one can attribute the presence of blood aggregation to the presence of an illness or a disorder. Therefore when RBC aggregation occurs, it means that the subject is at least not in perfect health. Red blood cell aggregation to shear stress Blood has a non-Newtonian fluid behaviour. Once it gets moving and builds momentum, it flows fast. Unlike water, which exhibits much more Newtonian properties, blood moves sluggishly at low speeds and is more liquid at fast speeds.
When the heart is resting between beats (diastole) it becomes more viscous and then when the heart forces blood out (systole), with the higher shear stress blood becomes less viscous. It is important to recognise that the presence of high RBC aggregation (along with hematocrit and plasma viscosity) is a factor contributing to low shear stress. Low shear stress leads to high whole blood viscosity.xxv
Eventually, elevated blood viscosity and RBC aggregation are important factors in affecting blood circulation, cardiovascular diseases and a number of diseases, 6as found in the cumulative work of renowned haemarheology (blood flow) researcher, Dintenfas.xxvi
Low shear stress regions along the arterial walls is associated with the occurrence of lesions.xxvii
This leads to deposit of plaque in these areas. When the plaque builds up, blood flow is further impeded, leading to turbulence and more plaque deposition. When the plaque deposition reaches a critical stage, it often causes vascular occlusive (blockage) disorderxxviii, and thrombosis (blood clot) both in arterial and venous systems. This is the prelude to cardiovascular events such as heart attacks and strokes. Before that happens, the subject exhibits classic symptoms indicated in TCM with blood stasis – such as reduced immunity, increased tiredness and so on, making the subject vulnerable to diseases.
Effect of RBC aggregation on blood circulation and tissue perfusion
There is considerable amount of research conducted by a core group of researchers on the effect of RBC aggregation on blood circulation and tissue perfusion.

The presence of RBC aggregation affects microcirculatory blood flow, venous flow, whole organ perfusion, and tissue hematocrit. Freedom from RBC would be a very desirable health condition. Red blood cell aggregation is an independent risk factor Studies also show that the presence of RBC aggregation is an independent risk factor for vascular occlusive disorderxxx and thrombosis both in arterial and venous systemsxxxi. With these properties, RBC aggregation would also be identifiable with poor circulation in the small capillaries. These studies show that RBC aggregation is also related to blockages in the larger vascular network, caused by the formation of plaques.

Blood is a specialized body fluid. It has four main components: plasma, red blood cells, white blood cells, and platelets. Blood has many different functions, including:
- transporting oxygen and nutrients to the lungs and tissues
- forming blood clots to prevent excess blood loss
- carrying cells and antibodies that fight infection
- bringing waste products to the kidneys and liver, which filter and clean the blood
- regulating body temperature
The blood that runs through the veins, arteries, and capillaries is known as whole blood, a mixture of about 55 percent plasma and 45 percent blood cells. About 7 to 8 percent of your total body weight is blood. An average-sized man has about 12 pints of blood in his body, and an average-sized woman has about nine pints.
Many people have undergone blood tests or donated blood, but hematology - the study of blood - encompasses much more than this. Doctors who specialize in hematology (hematologists) are leading the many advances being made in the treatment and prevention of blood diseases.

The liquid component of blood is called plasma, a mixture of water, sugar, fat, protein, and salts. The main job of the plasma is to transport blood cells throughout your body along with nutrients, waste products, antibodies, clotting proteins, chemical messengers such as hormones, and proteins that help maintain the body's fluid balance.Red Blood Cells (also called erythrocytes or RBCs)Known for their bright red color, red cells are the most abundant cell in the blood, accounting for about 40 to 45 percent of its volume. The shape of a red blood cell is a biconcave disk with a flattened center - in other words, both faces of the disc have shallow bowl-like indentations (a red blood cell looks like a donut).
Production of red blood cells is controlled by erythropoietin, a hormone produced primarily by the kidneys. Red blood cells start as immature cells in the bone marrow and after approximately seven days of maturation are released into the bloodstream. Unlike many other cells, red blood cells have no nucleus and can easily change shape, helping them fit through the various blood vessels in your body. However, while the lack of a nucleus makes a red blood cell more flexible, it also limits the life of the cell as it travels through the smallest blood vessels, damaging the cell's membranes and depleting its energy supplies. The red blood cell survives on average only 120 days.
Red cells contain a special protein called hemoglobin, which helps carry oxygen from the lungs to the rest of the body and then returns carbon dioxide from the body to the lungs so it can be exhaled. Blood appears red because of the large number of red blood cells, which get their color from the hemoglobin. The percentage of whole blood volume that is made up of red blood cells is called the hematocrit and is a common measure of red blood cell levels.
White Blood Cells (also called leukocytes)
White blood cells protect the body from infection. They are much fewer in number than red blood cells, accounting for about 1 percent of your blood.
The most common type of white blood cell is the neutrophil, which is the "immediate response" cell and accounts for 55 to 70 percent of the total white blood cell count. Each neutrophil lives less than a day, so your bone marrow must constantly make new neutrophils to maintain protection against infection. Transfusion of neutrophils is generally not effective since they do not remain in the body for very long.
The other major type of white blood cell is a lymphocyte. There are two main populations of these cells. T lymphocytes help regulate the function of other immune cells and directly attack various infected cells and tumors. B lymphocytes make antibodies, which are proteins that specifically target bacteria, viruses, and other foreign materials.
Platelets (also called thrombocytes)
Unlike red and white blood cells, platelets are not actually cells but rather small fragments of cells. Platelets help the blood clotting process (or coagulation) by gathering at the site of an injury, sticking to the lining of the injured blood vessel, and forming a platform on which blood coagulation can occur. This results in the formation of a fibrin clot, which covers the wound and prevents blood from leaking out. Fibrin also forms the initial scaffolding upon which new tissue forms, thus promoting healing.
A higher than normal number of platelets can cause unnecessary clotting, which can lead to strokes and heart attacks; however, thanks to advances made in antiplatelet therapies, there are treatments available to help prevent these potentially fatal events. Conversely, lower than normal counts can lead to extensive bleeding.
Complete Blood Count (CBC)
A complete blood count (CBC) test gives your doctor important information about the types and numbers of cells in your blood, especially the red blood cells and their percentage (hematocrit) or protein content (hemoglobin), white blood cells, and platelets. The results of a CBC may diagnose conditions like anemia, infection, and other disorders. The platelet count and plasma clotting tests (prothombin time, partial thromboplastin time, and thrombin time) may be used to evaluate bleeding and clotting disorders.
Your doctor may also perform a blood smear, which is a way of looking at your blood cells under the microscope. In a normal blood smear, red blood cells will appear as regular, round cells with a pale center. Variations in the size or shape of these cells may suggest a blood disorder.
Where Do Blood Cells Come From?
Blood cells develop from hematopoietic stem cells and are formed in the bone marrow through the highly regulated process of hematopoiesis. Hematopoietic stem cells are capable of transforming into red blood cells, white blood cells, and platelets. These stem cells can be found circulating in the blood and bone marrow in people of all ages, as well as in the umbilical cords of newborn babies. Stem cells from all three sources may be used to treat a variety of diseases, including leukemia, lymphoma, bone marrow failure, and various immune disorders.

New research may mark the expansion of a novel imaging agent for an optical technique called near-infrared fluorescence, which uses light energy to glean information about cells and tissues. NIRF combined with the newly synthesized agent can image dangerous blood clots hiding inside elusive veins, most commonly within the deep tissues of the thighs and pelvis, but potentially also in the coronary arteries. The agent uses a biomarker that seeks out a peptide (the building blocks of proteins) called fibrin that is actively involved in the formation of blood clots'.

The first known laser eye damage was caused by an infrared laser, a YAG laser at 1064 nm.
Infrared between 750 nm and 1400 nm will go through the cornea, lens, and vitreous and aqueous humors and arrive focused on the retina. Above a milliwatt or so, this can cause damage. Above 5 mW or so, it can cause almost instant damage.
At wavelengths longer than 1400 nm, the laser beam does not go through the cornea, so that it actually damages the cornea instead of the retina. But since it isn't focused by the cornea, generally higher powers are required to do damage.
I walked into a 10.6 micron carbon dioxide laser beam 40 years ago when safety was a bit below our standards today. It was about 100 watts per square meter or about 10 mW on my corneas. I suffered no damage but I certainly felt it and I was alarmed by it.
I have been around much higher power infrared lasers most of my life and I am very worried about COIL lasers because they have a 1315 nm wavelength. That means if I look at the target and light reflects from the target it could blind me. Even if an insect or bird flies through the beam, the scatter could cause eye damage to me. Why Heart Disease is on the Rise in America
Experts say the country needs to reverse trends like obesity and diabetes, or heart disease will continue to kill us and bankrupt our healthcare system.
It seems that Americans aren’t getting the message about heart disease.
New statistics predict that 45 percent of people in the United States will have at least one issue related to the disease by 2035.
That’s up from earlier predictions from the American Heart Association (AHA) of 40 percent by 2030.
The AHA predicts that costs related to the disease will double from $555 billion in 2016 to $1.1 trillion in 2035.
That “could bankrupt our nation’s economy and healthcare system,” according to AHA President Steven Houser, PhD.
He says heart disease and its complications are spreading quicker than originally thought.
Actor Bill Paxton died at age 61, reportedly from a stroke during heart surgery to fix a damaged valve, while fitness guru Bob Harper, from “The Biggest Loser,” suffered a heart attack at age 51.
Why is heart disease getting worse, even though our society seems obsessed with healthy living?
Other diseases like cancer and Alzheimer’s disease garner more attention, but the fact is cardiovascular disease (CVD) remains the nation’s most prevalent and costly killer, Houser told Healthline.
Even though smoking is on the decline, Houser said that other risk factors — obesity, poor diet, high blood pressure, and type 2 diabetes — are on the rise.
In addition, people may not realize how devastating heart disease is until they know someone who has it.
Dr. Michael Miller is a cardiology professor at the University of Maryland School of Medicine, and author of “Heal Your Heart: The Positive Emotions Prescription to Prevent and Reverse Heart Disease.”
He told Healthline that the main factors driving the rise in heart disease are obesity and type 2 diabetes, but the real underlying culprits are moving less and stressing more.
“What we aren't doing enough is getting up and out, spending quality time with loved ones daily, and smelling the roses,” Miller said. "We also need ‘me time’ to recharge, so I tell my patients to spend at least 15 minutes by themselves to collect their thoughts, whether by meditation or shutting off the radio while in the car.”
“I also suggest getting up every 15 minutes if you have a desk job and move around and stretch. Same recommendations hold while watching TV,” he said.
Dr. Regina Druz, a cardiologist at the Integrative Cardiology Center of Long Island, told Healthline that stress is a factor along with obesity, diabetes, and pollution.
“The epidemic is related to several factors, all converging in what amounts to a ‘perfect storm’ situation,” she said.
“However, what fuels the rise of diabetes and obesity, and heart disease down the line, is inflammation, and the impact of inflammation and environment on our genetics.”
Houser was surprised when he reviewed the recent AHA report due to the rising death rates and numbers that describe the financial toll CVD will have in the United States.
The report also finds that, by 2035, more than 123 million Americans will have high blood pressure, 24 million will have coronary heart disease, and more than 11 million will have experienced a stroke.
Houser said better investments in the National Institutes of Health (NIH) and Centers for Disease Control and Prevention (CDC) would reduce the burden and costs associated with heart disease.
Policymakers must invest in research and ensure Americans have affordable, quality health care--especially preventive care. Then, schools and workplaces have to create healthy environments that promote healthy habits, he said.
“The fact that CVD could singlehandedly bankrupt our nation’s healthcare system is disturbing,” Houser said. “But it’s a real possibility if we don’t act soon to do a better job of preventing what are largely preventable disorders."
